I irrevocably grant GSV Holding, LLC d/b/a Veritas Veterinary Partners (“Veritas”) or those acting on its behalf (“GSV”) my permission and the right to take photographs and/or video footage of me (“Recordings”) during this event.
GSV is the exclusive owner of all rights in the Recordings and I hereby irrevocably transfer to GSV my entire rights in the Recordings.
I irrevocably authorize and consent to the use and publication of the Recordings—whether in whole or in part—for commercial marketing and promotional purposes, educational purposes and internal communication purposes, including but not limited to social media, websites, email, printed materials and presentations, and any other lawful business purpose in perpetuity in all media, formats and technology.
TO THE EXTENT PERMITTED BY LAW, I HOLD HARMLESS, RELEASE AND FOREVER DISCHARGE GSV FROM ALL DEMANDS LIABILITY WHICH I, MY HEIRS, REPRESENTATIVES, EXECUTORS, ADMINISTRATORS, OR ANY OTHER PERSONS ACTING ON MY BEHALF OR ON BEHALF OF MY ESTATE HAVE OR MAY HAVE BY REASON OF THIS AUTHORIZATION.
I understand that the Recordings may be edited, copied, exhibited or distributed and waive any right to inspect or approve the finished product. I also waive any rights to royalties or other compensation related to the use of the Recordings and all other rights, including any privacy, publicity, moral or similar rights in respect of the Recordings.
BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS ABOVE, THAT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE GSV, AND THAT THIS CONSENT IS IRREVOCABLE.
